BENEFICIOS DE LA ALIMENTACIÓN ENTERAL PRECOZ FRENTE A LA TARDÍA EN PACIENTES PEDIÁTRICOS POSTOPERADOS DE ANASTOMOSIS INTESTINAL

Resumen

Introducción. La anastomosis intestinal pediátrica se asocia con complicaciones posoperatorias y hospitalización prolongada, por lo que el reinicio temprano de la vía oral podría favorecer la recuperación. Objetivos. Comparar los beneficios de la alimentación oral precoz (AOP) frente a la alimentación oral tardía (AOT) en pacientes pediátricos postoperados de anastomosis intestinal. Métodos. Estudio observacional, analítico, retrospectivo y de cohortes en 71 pacientes del Hospital San Bartolomé. La AOP se definió como inicio ≤72 horas y la AOT como inicio >72 horas. Se evaluaron dehiscencia anastomótica, vómitos, infección de sitio operatorio y hospitalización prolongada posoperatoria mediante historias clínicas institucionales. Resultados. Recibieron AOT 53 pacientes y AOP 18. No hubo diferencias en dehiscencia, vómitos ni infección. En el análisis crudo, la alimentación oral precoz presentó un menor riesgo de hospitalización prolongada en comparación con la alimentación oral tardía (RR=0,48; IC95%: 0,24–0,94; p=0,034) tras el ajuste la asociación perdió significancia (p>0,05). Conclusión. La AOP no incrementó complicaciones; su efecto independiente sobre la hospitalización requiere estudios prospectivos.
